Please complete the Department of Immigration Section 9 Application Form and attach a copy of the Section 9 form and a Statement of Need. The Statement of Need is a signed and dated letter from the sponsoring practice that identifies the need to hire a guest worker, locum tenens, or visiting practitioner. It should also include the proposed work permit dates for the employee.

Once the Section 9 form is completed by the Statutory Body, it is sent via email to the Department of Immigration. The Sponsoring Practice is included in this email. It is the Sponsoring Practice’s responsibility to provide the Department of Immigration with a hard copy of the completed Section 9 form once it’s been signed by the Statutory Body.

The Certificate of Professional Education Form (COPE) must be sent directly from the institution where you received your professional qualifications. (i.e. Degree, Diploma)
